Project Follow Up: PFU

What is Project Follow Up?
Project Follow Up: PFU offers high risk infants comprehensive developmental assessments at 4, 9, 15 and 24 months corrected age. Infants who are born with a risk factor(s) for physical developmental problems may qualify for this service. The goal is to identify infants and toddlers with delays and refer them for early developmental services to prevent serious physical disability. Infants who meet medical eligibility criteria are evaluated with no or minimal charge to the family. The assessments are performed at the CCS Medical Therapy Unit (MTU) by a pediatric physical or occupational therapist. Often, families are given limited advice regarding their child's physical development and beneficial physical activities. Reports are sent to the child's physician for review and appropriate action. Occasionally, a child and family are offered a visit at the CCS MTU Clinic to be evaluated by the MTU Team. This team includes the therapist, a pediatrician, an orthopedist, a social worker and a public health nurse.

Who is Medically Eligible for PFU?
In brief, an infant who has experienced a serious event that could lead to a physical developmental problem is likely to be eligible. Examples include infants who were born very prematurely or infants who had a very serious illness in the newborn period. Children who already have known developmental problems or neurological conditions are not eligible because they are referred to the CCS Treatment Program or other agencies for services. How do I obtain PFU Services?
To participate in the PFU Program, the family, the child's physician and CCS must complete the following steps:

  • Initial referral by hospital, physician, community organization or family (Click on CCS Referral Form)
  • Family submits application (Click on CCS Family Application)
  • Medical records are sent from hospital and/or physician to CCS
  • CCS reviews and determines medical eligibility
  • CCS contacts family to schedule an initial assessment at the MTU
