CHDP Provider Application & Certification
Who can be a CHDP Provider?
The State office of Children's Medical Services sets the standards for provider participation in the program and the counties manage the program locally. The State guidelines for CHDP providers are found in the CHDP Manual and are available on the State CHDP website. Click here for CHDP Manual. Qualified physicians, nurse practitioners and physician assistants may apply using the form available in the Manual Appendix.
What are the types of CHDP Providers?
There are two types: A "Health Assessment-only" Provider who performs the exam, is a Medi-Cal provider and maintains a medical record and a "Comprehensive Care" Provider who performs the exam, maintains a medical record, is a Medi-Cal provider on active status and acts as the primary health professional for the child's health care. The Comprehensive Care Provider is reimbursed at a higher rate for CHDP exams.
Who certifies a CHDP Provider?
The Santa Barbara County CHDP Administrative office certifies providers who will deliver CHDP services in Santa Barbara County. The State of California issues the provider a CHDP Provider number that must be used when billing for services. A CHDP Provider number is unique to the provider and to the location of the provider's practice. Therefore, a provider needs a distinct number for each location of practice.
What is the CHDP Provider Agreement?
The CHDP Provider must agree to provide all the required CHDP services as defined in the CHDP Provider Manual, CHDP Heath Assessment Guidelines and regular CHDP Program Information Notices and communications. These documents describe who is eligible for CHDP services, the type of service required based on the age of the child, frequency of CHDP services, follow up responsibilities of the Provider and reimbursement procedures.
How do I become a CHDP Provider?
All potential applicants should review the CHDP Provider Manual on-line to understand the intent and operation of the program. The appendices contain the provider application and agreement along with instructions. The
application and agreement should be mailed or sent by FAX to the Santa Barbara County CHDP Administrative Office (address below). If a potential applicant wishes to discuss the program and its requirements with one of our staff members, please call the number below.
