Eligibility and Accessibility
Who is eligible for a CHDP Examination?1.All children and youth from birth through 20 years of age who have full scope Medi-Cal (Medi-Cal that covers all medical costs) are eligible for a CHDP Examination.
2. All children and youth from birth through 18 years are also eligible if they: 1) do not have full scope Medi-Cal; 2) are not covered by insurance that provides preventive services; and 3) meet the family income eligibility criteria, which means that the family makes less than 200% of the federal poverty level - this amount changes every year. Children whose families meet the CHDP income eligibility criteria may receive up to two months of full-scope Medi-Cal coverage. Families will be given the opportunity to request an application to enroll their children in either Medi-Cal, or the Healthy Families Program .
What
does a family need to bring to a CHDP visit to confirm eligibility?
If your child has a BIC card issued by the state, be sure to bring it to the doctor's office.
Families of children who do not have full-scope Medi-Cal will be asked by the doctor's office to complete and sign a one-page form called the DHS 4073, that states their income.
If my child receives temporary full scope Medi-Cal, how do I use it?
If you are given temporary full scope Medi-Cal for your child at his/her CHDP visit, you will be given a paper that has your child's full scope Medi-Cal number printed on it. This paper with the Medi-Cal number can be used right away to get Medi-Cal services for your child. Within one week, you will get in the mail a plastic card called a “BIC Card” that you may show to any Medi-Cal provider (pharmacy, dentist, doctor, laboratory) to have the cost of your child's care paid for by Medi-Cal. The provider will use a computer to make sure that your child can get services with this card.
If my child received temporary full scope Medi-Cal, how can I prevent it from running out?
At the time of the CHDP exam, you will be asked if you want to receive a Medi-Cal & Healthy Families insurance application. If you say yes, an application will be mailed to you within two weeks. When you receive the application, you may fill it out and return it in the envelope that came with the application. When the Medi-Cal office receives your application your child's temporary full scope Medi-Cal will continue until the application is read and you receive a letter saying whether or not your child can have Medi-Cal or Healthy Families. When this health insurance is given, you can decide if you want this insurance. If you decide that you do, your child's coverage will continue.
