venom allergies, asthma, allergic rhinitis (runny nose), hay fever

Description of Services: Care for
patients with rheumatoid arthritis, severe osteoarthritis, lupus,
scleroderma, or Reiter's Syndrome. Appointments only with referral
from primary care provider. New patients: all pertinent x-rays
and medical records should be brought to the appointment. Referral
to this clinic is by primary care provider or physician or the
Public Health Department.

Clinic
Description of Services: Care for
patients with dermatological problems. By appointment or referral
only.

Endocrinology
Clinic
Description of Services: Care for
patients with endocrine disorders such as thyroid problems and
Cushion's syndrome. New patients: all pertinent medical records
and lab results should be sent to the clinic prior to or at the
time of appointment. This is a teaching clinic. All new patients
will see Resident first. Plan to stay 2 - 3 hours. Referral to
this clinic is by internal medicine or primary care physician.

Family Planning
Clinic
Description of Services: Services
for men and women including contraceptive information, education
and women's health annual exams. Office visits include pap smears,
birth control, sexually transmitted infections, Hepatitis B screening
and vaccinations, abnormal pap smear follow-up and tubal ligations.
Services are targeted for Spanish speaking; homeless; geographically
isolated populations; teens; and low-income individuals who want
personal choice in determining the number and spacing of their
children.
Patients may qualify for the PACT program - a state funded program
providing fee exams and birth control methods to men and women.
Family planning services are provided at the following clinic

Gynecology
Clinic
Description of Services: Care for
adults who require gynecology services which include pap smear
and colposcopy. Nursing consultation required prior to scheduling
appointment for tubal ligation. There are no infertility appointments.
New patients and clients referred from outside the Public Health
Department should have all pap smear results and pertinent records
sent to the clinic prior to or at the time of the appointment.
Appointment or referral may be required for this clinic.

Hand Clinic
Description of Services: Care for
patients with problems from elbow to hand; skin infections of
the hand; arthritic deformities; laceration; tendons, nerves,
Carpal Tunnel, etc. No Worker's Compensation cases are seen in
these clinics. Referral to this clinic is by emergency room MD
or RN. All x-rays and medical records should be brought to the
appointment. These services are provided at following clinic sites
throughout Santa Barbara County.

H.I.V. Testing
Anonymous & Confidential HIV testing.
May be self-referred.

Immunization Clinics
The Public Health Department provides education, outreach and
immunization for infants and toddlers to meet the goal of immunizing
90% of two-year olds. There are special programs that clinics
use to help pay for vaccines. For those patients who do not qualify
for these programs, there is a $15 charge which covers all vaccines
given. No one will be turned away due to inability to pay.

Infectious Disease
Clinic
Description of Services: Care for
adults and children who have problems which include positive HIV,
symptoms of AIDS, Lyme's disease and other infectious diseases.
Self-referral with positive HIV and symptoms of AIDS, or may be
referred by a physician or RN.

Description of Services: Care for
adults and children ages 15 and older who have problems which
include heart disease, liver disease, thyroid disorders, gastro-intestinal
disorders or diabetes.

A clinic social worker assists patients and families with identified
needs to prevent disability, enhance health and maintain independence.
Social workers are available for clinic patients and their families.

Description of Services: Care for
patients with kidney problems. Referral to this clinic is by physician
only.

Neurology
Description of Services: Care for
patients with neurological disorders such as seizures, numbness
of body, migraine headaches, multiple sclerosis and cerebral palsy.
New and returning patients must bring all pertinent x-rays, diagnostic

Public Health
Nursing (PHN)
Public Health Nursing is the practice of professional nursing
in the community. The Public Health Nurse (PHN) is a member of
the health team which includes service aides and health educators.
The team works to improve community health by performing the core
public health functions of assessment, planning, intervening and
evaluating, policy development and quality assurance. Services
are provided at three levels; individual, family and community.
The ultimate goal is to prevent disease, promote health, alleviate
adverse conditions that affect health and safety and encourage
wellness and independence.
PHN outreach, TB follow-up, referrals, prenatal and postpartum
visits are made by the district PHN and a Spanish speaking or

STD Clinic
(Sexually Transmitted Diseases)

(Sexually Transmitted Infections)
Description of Services:
Sreening for male and female patients for sexually transmitted
infections. All patients are given the option to be screened for
Chlamydia, Gonorrhea, Syphilis, and HIV. Patients are visually
screened for HPV(Human Pavillomavirus), HSV (Herpes Simplex Virus),
Trichamoniasis, yeast and bacterial vaginal infections.
Clients may qualify for the PACT (Family Planning) program. If
not, at the time of service patients are asked to pay a set fee
of $20. This fee pays for the exam, lab tests and medications
for treatment of infections and a two-week follow-up visit. No
charge for contacts with referrals.

Surgery Clinic
Description of Services: The Public
Health Department Surgery clinics provide outpatient ambulatory
care for adults and children who have problems which include hernia,
gallstones, breast lumps, masses, rectal bleeding, hemorrhoids,
lacerations, post-op care and proctosigmoidoscopy. New patients
must bring all x-rays and medical records if available.

These services are provided at the following clinic sites throughout

Urology Clinic
(Urinary Bladder)
Description of Services: Care for
patients with chronic urniary tract infections, prostate problems,
bladder cancer and vasectomies. No impotence services are provided.
These urology services are provided at the following clinic sites
throughout Santa Barbara County. Referral to this clinic is by
physician or emergency room.

Well Child Clinic
(Healthy Families )
(Child Health and Disability Program)
Description of Services: Complete
health assessments for the early detection and prevention of disease
and disabilities. They are provided for children and youth. Medi-Cal
recipients from birth through 20 years and children from birth
through 18 years whose families meet income standards of up to
200% of poverty level are eligible. Assistance in obtaining diagnostic
and treatment services found to be needed during the health assessment
is also available. Services are available in Spanish.
Women, infants and children (WIC) nutritional referrals.
Child Health and Disability Program
CHDP Teen Clinic
These services are provided at the following clinic site/s:
The visit includes CONFIDENTIAL teen class and doctor's
exam.
