Program Information
There are two programs within
the Public Health Department that provide services for the older
adult population.
The Multipurpose Senior
Services Program (MSSP)
is a community-based social and health care case management
program that assists the frail elderly to remain safely in their
homes, thus delaying or preventing institutionalization. Case
Managers perform in-home assessments, prepare individual care
plans, purchase services which enable the person to remain at
home, and monitor the effectiveness of those services. This program
provides services to approximately 200 clients per month.
The Geriatric Assessment
Program (GAP)
provides information, referral, consultation in-home assessments,
care planning, short-term case management, senior advocacy and
community education for the frail, dependent adult at risk of
institutionalization.
