Eligibility

The services of MSSP are available to frail, elderly persons living in Santa Barbara County. Participants Must:

• Be age 65 or older

• Be recipients of Medi-Cal benefits

• Be at risk of being placed in a nursing facility

• Have a need for case management

• Have limitations preventing them from performing some of the activities of daily living (i.e, bathing, grooming, transferring, eating, toileting).
