Multipurpose Senior Services Program

The Multipurpose Senior Services Program (MSSP) identifies the health and social needs of older, frail people who are at risk of nursing home placement. The goal is to provide high quality case management in order to:

• Avoid costly, premature or inappropriate nursing home placement

• Identify and coordinate the appropriate levels of care and services which will enable the frail and elderly to maintain their independence at home.
