The core of MSSP case management is a team consisting of a social worker public health nurse, health service aide, and supervisor. Staff will:
Perform a full assessment of the client's
health and social needs
Develop a care plan including direction from the client, physician and family
Develop a care plan including direction from the client, physician and family
Provide timely, effective and efficient linkage to services needed to meet the client's care plan goals
Perform ongoing review of the client's needs to monitor progress or identify further unmet needs
Advocate on the client's behalf.
As funding permits, purchase additional services from local vendors.
