Who are we?
The Geriatric Assessment Program is comprised of a team of Public Health Nurses who have specialized training in the field of aging. A Psychiatrist with Board Certification in Geriatric Medicine completes the team of professionals.

Who do we serve?
Residents of Santa Barbara County who are 60 years of age or older may be eligible for services. Referrals can be made by the client, family, neighbors, friends, physicians or other professionals.

What do we do?
The team members assess the client who may have difficulty maintaining their independence. A care plan is developed and interventions recommended. A consultation may be provided to the family, caregivers, and professionals which enables the older person to safely remain in the least restrictive setting.

How do we do it?
The Geriatric Assessment Program does a complete assessment by a trained professional. The assessment covers:

• Health
• Psychological Factors
• Financial/Legal Issues
• Social Supports
• Activities of Daily Living

This information is reviewed by the team and a treatment plan is developed that best suits each individual client. The Geriatric Assessment Program works closely with community agencies serving the elderly. This enables the team to put together support systems to keep people in the home setting as long as this is a safe and reasonable option. The Geriatric Assessment Program staff offers family meetings for clients and/or caretakers to enable them to better understand the problems and diseases the family is facing.

Where do we provide the service?
The Geriatric Assessment Team makes home visits to assess the client in the home setting.

Who are the support systems?
The formal support system may be agencies providing medical care, in-home services, or home delivered meals. There may also be informal support such as family, neighbors, friends or clergy. The team helps coordinate the formal and informal care system to enable a person to maintain a quality of life that is safe and humane. The Geriatrics Assessment Program does not purchase services on the clients behalf.

How much does it cost?
There are not charges for geriatric assessment services; however, donations are accepted.
