Chronic Disease Self-Management Program

Nearly 100 million Americans suffer from such chronic illnesses as diabetes, arthritis, respiratory problems and heart disease. Living with these and other chronic illnesses is a daily test of emotional and physical endurance. And although chronic illnesses and their limitations must be endured constantly, learning to manage and control the symptoms and conditions can enable you to live a fuller, happier lifestyle.

Why a self-management course for people with chronic conditions?

While there are many chronic conditions, the problems and symptoms associated with each are often similar. Many people with chronic conditions suffer fatigue and loss of energy. Some experience pain, while others have breathing difficulties. Problems with sleeping and depression are common, combined with concerns for the future. This course is specifically designed to help individuals develop the tools necessary for living a healthy life while managing the symptoms associated with chronic conditions.

What does the course cover?

You will learn self-management skills needed to:

• Manage your symptoms
• Communicate effectively with your doctor
• Use relaxation techniques
• Fight fatigue
• Make daily tasks easier
• Set goals and solve problems
• Lessen your frustration
• Work with negative emotions
• Get more out of life

The course is for people of all ages who have one or more chronic conditions. A friend, caregiver, or relative is welcome to attend the coruse with you. They too, will benefit from the self-management tools taught in this program.

Who can participate?

The Chronic Disease Self-Management course is appropriate for persons with the following diagnoses:

• Heart Disease
• Arthritis
• Stroke
• Diabetes
• Bronchitis
• Emphysema
• High Blood Pressure
• And other related diseases

Classes are offered at locations throughout Santa Barbara County.
